Now more than ever, palliative care is emerging as a key element in the successful navigation of the care continuum. Whether you are an experienced palliative care provider or simply looking to learn more about the future role of palliative care and serious illness programs, this collaborative conversation will allow you to hear from your peers, sharing successful approaches to advanced care planning, proactive health management, and  providing the right kind of support for patients and their families, in the right care venue, at the right time. 

Networking sessions offer attendees an opportunity to talk with their peers while learning from session facilitators. During these sessions, facilitators will alternate between concise presentations of educational content and small group breakouts, allowing attendees to engage with one another and discuss the ideas presented. These sessions will use virtual Zoom breakout rooms, and we encourage participants to turn on their cameras to facilitate conversation and connection. A Zoom link will be  provided to registrants.
