Post-Acute Care providers continue to face regulatory uncertainty. The shift to value-based care in the post-acute care segment is proceeding however, it appears that designing a unified, equitable, and appropriate PAC payment system is a bit more challenging than expected. Questions of payment incentives based upon reporting factors under a provider’s control may be vulnerable to misreporting.

The effort to reduce variation and cost across the post-acute continuum remains a policy goal, but the recognition that a basic care payment model for similar patients across all PAC providers requires a second tier model to address the specialized high care needs and providers would be required to meet each set of condition specific requirements.
Proposed legislation continues to emphasize the monitoring and reporting of standardized patient assessment data elements (SPADEs) across all patients. The addition of these SPADEs to the IRF-Patient Assessment Instrument (IRF-PAI) address improvement in coordination of care and facilitate communication between PAC providers and other members of the healthcare community.
