Several post-acute care EHRs are tailored for hospice care, but agencies that also deliver palliative care need a software solution that bridges the gap. This session will cover how Consolo Palliative software provides an application tailored for palliative programs by encompassing many of the data elements adapted from hospice, while also interweaving the palliative specific documentation/reporting needs. From care planning to CPT coding, we will journey through the pain points faced by palliative programs and their mission to select a palliative EHR and offer solutions to assist this evolving specialty in achieving their mission to care for the chronically ill.
