Description
When Reading (PA) Hospital identified food insecurity as a major problem in the community, the organization took a proactive and collaborative approach, creating clinical-community linkages with food banks, community-based organizations, and the public to improve food access. Reading also collected a wide range of data — including the effect of food access on readmission, A1C measures, and access to primary care. Examine the tactics taken and results achieved that greatly improved wellness in the community.
