A growing evidence base has emerged documenting that nonclinical factors – such as unstable housing and food insecurity – can have a major impact on health outcomes and costs. With this in mind, health services payers and providers are increasingly looking for opportunities to address these “social determinants of Health” (SDOH), particularly for high-need and high-cost populations. This focus on SDOH creates new, and promising opportunities for partnerships between health services payers and providers, and their human services counterparts. This session will take a look at how the focus on SDOH is changing the way health services payers and providers think about, and pay for care; the challenges and opportunities for partnership across the health and human services sectors; and how human services organizations can best position themselves to craft partnerships that align with their missions, support high-quality services, and leverage sustainable funding solutions.
