Description

Over the past several years, there has been growing interest across the healthcare continuum on closing the loop on referrals to community based organizations.  Health care organizations are wanting to more easily identify and refer patients to social service organizations – the primary providers of services targeting the Social Determinants of Health.  This session will provider participants with an overview of closed loop referrals processes, describe how CLR functions differently from traditional Information & Referral/ADRC.  Participants will have an opportunity to learn from national experts and peers on opportunities, challenges and successes in closed loop referral processes.

Networking sessions offer attendees an opportunity to talk with their peers while learning from session facilitators. During these sessions, facilitators will alternate between concise presentations of educational content and small group breakouts, allowing attendees to engage with one another and discuss the ideas presented. These sessions will use virtual Zoom breakout rooms, and we encourage participants to turn on their cameras to facilitate conversation and connection. A Zoom link will be  provided to registrants.
