This presentation will share how a continued effort to bring together community-based organizations and technology (including the HMIS) can assist with closing the health disparity gap. Paige Stanton will review how building a community model that integrates technology and analyzes data can drive process and ultimately, change in a community. We will look at how St. Johns County implemented this model and the outcomes as a result of the work being done there.
